3. Interested eligible candidates should send their duly filled applications (only in the prescribed

format) along with self-attested copies of the relevant supporting documents/certificates if

required by speed post/courier to Registrar, Chief District Medical & Public Health Officer,

Gajapati, AUPo-Paralakhemundi, Gajapati, 761200 on or before 09 April 2026.
