3. Upload photo/signature: Option 1 (single A4 scan, 200 DPI, <150KB) or Option 2 (separate JPEGs: Photo 238x306px <50KB, Signature 350x150px <30KB). Recent photo essential.
4. Preview form, submit—corrections allowed till March 8 (3 PM).
5. Download acknowledgment; print and send hard copy with self-attested copies (Birth/SSLC/HSC/Diploma marks, Midwife registration, Employment Exchange card) to: The Director, DHFWS, Victor Simmonel Street, Old Maternity Hospital, Puducherry-605001 by March 11 (5 PM) via Speed Post/In-person. Superscribe "APPLICATION FOR RECRUITMENT TO THE POST OF ANM".
